Please note that prior to scheduling a Psychosexual Evaluation, the assessment fee must first be paid/received. This fee includes time spent for the assessment, interviewing, and the actual report.
To make a payment, you may go online at: www.texaspcs.org and click on the “Pay Now” button. Once payment is submitted, you should promptly receive a receipt via email. Please note that in lieu of the Online Payment Option, you may elect to send a check or money order to the Mailing Address listed below.
Although a referral from a doctor is not needed, we do request that you please provide the name of your Attorney and/or Probation Officer, if applicable. In addition, we would require a copy of any associated police report(s) as well. Furthermore, if you have a current therapist, a Release would be required in order that Ms. Logan may speak with them as well.
The final report typically takes about six weeks to complete, and are administered in our Frisco Office location. After the report is completed, we wlll send the report to your attorney and/or Probation Officer. Therefore, please remember to provide your their contact information including phone number and email address, along with any associated Police Reports.
We have a 48-hour cancellation policy. If you miss your appointment, or cancel with less than 48 hours’ notice, you will be charged the full price of your assessment.
